Branch Officers Update Form: Use this form to update your Branch Officers with ESU Headquarters.  

ESU Deceased Members: If you would like your deceased members to be remembered by the ESU, please fill out this form. We thank you for taking the time to provide some information regarding your Branch's recently deceased. We are saddened by the news of their passing and hope to memorialize them with the information you provide. 

Request for Certificate of Insurance for Branch Event: Please fill in the details of the event for which you would like a Certificate of Insurance issued. This online form will be submitted to Grant Hamel at ESU US National Headquarters.  Please direct any questions to Grant Hamel. Please note that a Certificate will not be issued to a Branch delinquent in submitting to the National Headquarters the annual insurance premium of $250.
